Medicare Rebates
You may be eligible for up to 5 Medicare rebates of $63.40 within a 12 month period, through a Chronic Disease Care Plan from your GP.
A copy of your plan must be provided prior to your appointment to allow for the automatic processing of Medicare rebates.
I am able to see both plan and self-managed NDIS participants. As per the NDIS price guide, participants will be charged at $178.99 per 60 minutes. Additional fees will apply for clinical documentation, liaison with care team and report writing ($178.99 per 60 minutes). Ensure you select "NDIS" appointment link when booking online.Â

Check with your individual health fund to see if you have extras cover for Dietetics.Â
A receipt will be emailed to you following payment which contains all the relevant information for you to submit your claim.Â
